1. Signs it is time to start, before a fall
- Standing up from a chair needs hands or several rocking attempts
- Noticeably slower walking, short steps, holding walls or furniture
- Stopping to talk because walking and talking at once is no longer possible
- Knee or hip pain on stairs, leading to avoiding going out
- A fall or near-fall in the past year and growing fear of walking
These are the moments when training still works well, because strength and balance can still recover. Waiting until a fall means recovering from injury and fear at the same time.
2. What the physiotherapist does at home
The first visit is an assessment: leg strength, balance, walking speed and a timed sit-to-stand-and-walk test used worldwide to gauge fall risk, plus a look at the home for stairs, bathroom, thresholds and lighting. Training then happens in the real environment: getting out of the actual bed, walking the real distance to the bathroom, climbing the home's own stairs. Between visits, the therapist leaves a safe daily routine and teaches the family to supervise form; most of the result comes from consistency between visits.
